NURS FPX 4030 Assessment 4 Remote Collaboration and Evidence-Based Care

NURS FPX 4030 Assessment 4 Remote Collaboration and Evidence Based Care

Remote Collaboration and Evidence-Based Care

In my talk I will address the role of remote communication means in healthcare as one of the most important elements of it. This method is an innovative way to bring healthcare professionals to collaborate on virtual patient cases. Technology is key for this process to work; it is the glue that brings all parties involved in healthcare together at some point. Ex. In Caitlyn’s case, a 2-years-old young lady with cystic fibrosis, remote collaboration can serve as a possible solution that can enhance her access to healthcare services and give support to her treatment. Due to the COVID-19 pandemic, telehealth has gained significant popularity since it helps to reduce health care costs, enables better patient access to healthcare services, and improves patient care. (Kichloo et al., 2020).

The panel of doctors can lean on telehealth applications, the asynchronous exchange of data, symptoms, and emotions in Caitlin’s care. Moreover, the use of electronic health record (EHR) portals has given healthcare providers efficient ways to work apart and share vital patient pieces of data (Gajarawala & Pelkowski, 2020). Healthcare professionals will be able to leverage these remote collaboration tools, and the primary care physician will be able to synthesize the information they received from the experts and develop a coherent course of treatment for Caitlyn that will substantially improve her health and make it easier for her manage her cystic fibrosis (Kichloo et al., 2020).

Evidence-Based Care Plan and Patient Safety

Scenario for Remote Collaboration

Caitlyn, a little girl aged 2 has spent about 30 days a year as an inpatient battieling pneumonia. Her health condition recently gave away a diagnosis with the said disease and thus the plan with her family that entails on how they will carry their healthcare process is in order. Caitlyn’s parents live too far away from the hospital, and because of their divorce, they seem to be even incapable to fix their issues. As a result, the Nurse Anderson and Dr. Copeland have analyzed Caitlyn’s medical report and thus developed a personalized patient care plan by taking into account Caitlyn’s family’s unique set of conditions. Our mission is for the family to come with improvements which are being followed easily and enabling Caitlyn to receive the care she requires with no hindrances. The plan calling for resource use to be optimised and exploring avenues to enable parents to provide better care for to their children.

Evidence-Based Care Plan

Evidence-Based Care Plan
The cystic fibrosis is a genetic disease that mostly manifest in lung and digestive tract symptoms like recurrent lung infections, shortness of breath, and poor reduction of body weight (Blanco et al., 2022). When Caitlyn is a medical issue of a two-year-old with cystic fibrosis, telemedicine can be an ideal solution. Here is an evidence-based remote care plan for Caitlyn:Here is an evidence-based remote care plan for Caitlyn:

In this plan, it is hoped that Caitlyn will be taught to improve her case through the use of evidence-based strategies, the best of which are chosen. Care of Caitlyn will, therefore, be advantageously implemented through telehealth and remote cohort. Interdisciplinary collaboration is an essential means of controlling cystic fibrosis leading to quality patient care. Caitlyn will be under the medical supervision of a pulmonologist, physiotherapist, dietician, and respiratory therapist, and each of their respective members will work in cohesion in order to provide top-notch care to Caitlyn (Blanco et al., 2022). They will work jointly and utilize the telehealth partnership to interact with Caitlyn and her parents. Since telehealth allows the medical team to remotely evaluate and supervise the patient, the collaborative effort of the multidisciplinary team can ensure necessary and adequate tracking and progress evaluation of the patient with the condition.

Without any doubt, the VR pulmonary rehabilitation and physiotherapy will be used to assist Caitlyn’s recovery. During virtual sessions a physiotherapist will talk Caitlyn through exercise that to aid in her lung function and muscle strength improvement. The training will take into consideration her objectives and her requirements (Darabseh et al). The pulmonologist will pay due diligence to Caitlyn’s respiratory health and offer a medical plan including the right drugs and therapy. The respiratory therapist gives Caitlyn training in different airway clearnance techniques and also guides her through pulmonary rehabilitation process. Caitlyn’s dietitian will evaluate her nutritional status and create a nutritional plan to suit her unique preferences. The specialist will guide Caitlyn through where she needs to exercise to improve her lung function and strength.

Educational resources and online workshop materials will be shared with Caitlyn’s parents to aid her in overall conditioning during re-visits. These resources should provide an overview of cystic fibrosis management, how airways should be cleared, the patients’ diet, and the necessity of fitness (Albon, et al., 2022).

Caitlyn can benefit from telemedicine as it provides access to appropriate medical care without having to leave home. This innovative approach can enhance the quality of Caitlyn’s treatment and mitigate the possibility of errors or misunderstandings in the medical diagnosis or treatment. Besides, regular interaction and teamwork among healthcare providers can improve patient outcomes and increase satisfaction (Kichloo et al., 2020).

Evidence-Based Practice Model to Develop Care Plan

To Accomplish these, the Professionals will emuluate the Evidence-Based Practice Model to Develop Care Plan.
The driving force for Caitlyn’s remote care plan was the Johns Hopkins Nursing Model, which is evidence-based and is also frequently quoted by the nurse (Tsistinas, 2020). Within this approach, there are three steps that allow evidence-based practice to be implemented: asking the Question, Finding Evidence, and Translation.

Practice question: Identification of a critical or clinical issue or practice question is the first step of the process. Keeping in mind the medical concerns or demographic of interest, the question must be specific, measurable and have a resolution (Tsistinas, 2020). This could include a discussion on diagnostic processes, treatment options, prognosis or any other health related issue.
Evidence: The second is the search for and the careful assessment of the evidence from ecfficient articles to respond to the practice question. Such would require a systematic evaluation of the existing literature using controlled experiments, observational studies, cross sectional or other studies (Liza and Kaiser, 2021). Whether the evidence of quality is based on, for exam, study design, sample size, and risk of bias should be seriously considered.
Translation: The culminating move is to apply the evidences in the field practice. It could include the design and implementation of clinical protocols, algorithms, and guidelines either using the evidence or based on it. It can also be conducted through the training of health care professionals on how to utilize evidence-based practices. Alternatively, there can be a shift in organizational standards or policies to enable implementation of evidence-based practices during that period (Liza & Kaiser, 2021).
Through the powerful application of the PET process healthcare providers can make appropriate decisions based on available evidence to improve patient areas and provision of health services. The model focuses on the usage of the deepest knowledge to choose the right treatment and better the patient’s outcome (Tsistinas, 2020). In the Johns Hopkins model evidence-based approach consists in the question on clinical practice, a search and the appraisal the relative evidence and translating the findings into practice (Tsistinas, 2020)… This technique is very helpful in biomedical research in the sense that it provides a lot of information on the behavior of human beings.

How to Evaluate Positive Benefits for Patient Outcome

The benefits of Caitlyn’s story in telehealth for people with cystic fibrosis can be evaluated by satisfaction surveys and tracked also her health outcomes over time. The healthcare staff will be checking the number of her hospital visits, and her parents will also be participating to contribute with the effects of the plan and our healthcare services (Lukewich, et al., 2022). Besides, they will monitor her treatment adherence. Also they will track her medication regime and lung function and overall health condition to have an idea of the results of the telemedicine program. With the factors discussed above, consequently, we can detect the quality of telehealth in handling cystic fibrosis and the final outcome using the guideline (Lukewich et al., 2022).Reflection on the Relevance of the Evidence

Among the literature authored on telemedicine and virtual rehabilitation on CF in children, the most reliable and applicable research evidence was gathered on the treatment plan. From the Google Scholar, PubMed, and CINAHL databases, sources that are considered reliable sources, we learned that the telemedicine mode has widened access to care, reduced hospitalizations, and favorably impacted the patient´s outcome among children with cystic fibrosis (Ferro et al., 2021). This work by (Vagg et al., 2021) indicate, that virtual treatment is actually a tool, which surely improve the life, activity level and lung function in children suffering from cystic fibrosis. Telemedicine has been shown to be an impactful and excellent alternative to traditional in-person treatment approaches that enable many kids to attend more frequent and convenient sessions that are adapted to their specific needs, thus, the burden of their disease management diminishes and leading to healthier outcomes eventually. With the aid of telemedicine and virtual rehabilitation, the treatment of children with cystic fibrosis can gain some traction, and it is an approach that would sit well with the care plan and be suitable for remote collaboration (Ferro et al., 2021).

The Rationale to Determine Relevance

It should be noted that making the distinction between the reliability and application for a particular situation is contemplated to be assisted by a method or set of questions (Central Michigan University, 2022). Another framework that is frequently used is the CRAAP test, which is a sum of currency of the source, relevance of information, authority of the source, accuracy of the information and purpose of table of findings (Kurpiel, 2023). The reliability of data as the basis of the plan for the treatment is unavoidable, because the process of searching for the treatment in cystic fibrosis is in process all the time (Central Michigan University, 2022).

Strategies and Benefits of Interdisciplinary Collaboration

Cooperating among various individuals in healthcare is the key to giving the patients holistic and individualized treatment. Practically by cooperating with specialized medical personnel from different industries, they are able to distribute knowledge and skills through, thus it becomes easy to keep in observations and see to it that patients receive holistic precision health care which actively involves their unique needs. Remote communications through telephone has been taking its momentum and at the same time, telehealth offers a lot of benefits to healthcare providers. Among her healthcare contributors, Caitlyn is receiving mental nursing, medical intervention, physiotherapy, and social work from the team, which may or may not be aided by remote communication to provide her the best care (Bendowska & Baum, 2023). Yet such project can be difficult when it comes to the interdisciplinary collaboration between remote teams. Such as team members unable to articulate their position or understand their role in the team which results in miscommunications and a less efficient function.

Strategies to Mitigate Challenges

For this matter, healthcare providers can work on caring culture and collaboration, as well as a way to organize the teams and use team expertise to overcome any challenges ahead of them. Encouraging a culture of compassion and mutual care should be those factors that facilitate the environment where staff understands that only coordinated work and effective communication help to solve problems (Bendowska & Baum, 2023). The gap can be filled through the provision of regular training, the creation of platforms for team members to cement together and also comprehend each other’s roles and functions.

Interdisciplinary Collaboration as Leverage for Improved Care

This will be through encouraging a cadre of care which will further nurture cooperation. An integral part of interdisciplinary collaboration is joint efforts and constructive communication which can be accomplished by creating a culture of care within a team structure that pays attention to collaboration and teamwork (Bendowska & Baum, 2023). A second way in which the creation of a cooperative organization can be achieved is the coordination culture, which will ensure the necessary collaboration between an interdisciplinary team (Bendowska & Baum, 2023). To conclude, using teamwork and professional competencies to realize the best interprofessional collaboration in the healthcare field (Bendowska & Baum, 2023) could be efficient.

Conclusion NURS FPX 4030 Assessment 4 Remote Collaboration and Evidence Based Care

Conclusion As technological advancements stand, the face of modern healthcare is set to become more patient and more community-centered with the application of digital tools in patient care.
Telemedicine can help Caitlyn with cystic fibrosis. Cystic fibrosis is an inherited chronic disease which can’t be cured. It disposes of the necessity of physical visits thus, reduces the distance to a specialist. Disruptions of communication and transportation too are avoided. Discipline teams in the center of care are able to collaborate which leads them to share personalized care and health information. Through telemedicine, Caitlyn’s personalized care can be gave attention by considering her specific needs and preferences which may lead to improvement in her health.
